Technical and supportContact page. Response targets are in the uptime and service level summary.
Accessibility issuesAccessibility statement, which sets out what happens after you report something.
Security and vulnerability reportsThrough the contact page. We will fix it and credit you if you want the credit.

3. Registrations: what we hold, and what we do not

Listed both ways. If a missing item is a submission requirement for your solicitation, you need to know now rather than at award.

ItemStatus
Massachusetts LLC registrationCurrent. Verifiable on the Commonwealth's corporate database under the legal name above.
Form W-9Current and signed. Sent on request, usually the same day. Not published, because it carries a taxpayer identification number and a signature.
EINHeld. Supplied on the W-9 rather than on this page, for the same reason.
SAM.gov registration and UEINot registered. Required for federal contracting and cited by some state and local programmes. If your solicitation requires an active SAM registration at submission, we cannot meet that today. Ask us — registration takes weeks, not months, and a live opportunity is a reason to start it.
CAGE codeNone. Follows from SAM registration.
DUNS numberNone. Obtainable on request if your process requires one.
State and municipal vendor registrationsNone yet. We will complete yours as part of responding. Tell us which portal and what it needs.
Certificate of insuranceNot yet available. General liability, cyber liability and professional/errors-and-omissions cover is being placed. Until the policies are bound there is no certificate, and a certificate is the only acceptable evidence of cover.
Minority, woman, veteran or disadvantaged business certificationNo certification claimed. We hold no certified status under any such programme and will not imply one.

4. References

We have no public-sector references yet. This is a new line of business and the first agency to hire us will be the first.

What we can offer is a public institution — a physiotherapy school — whose clinical teaching tool we built, and who can be approached for a reference with their permission asked first. The work itself, including what was built and why, is on our case studies page. If your solicitation scores three comparable agency references, say so when you write to us and we will tell you honestly whether it is worth your committee's time.

5. Capacity and continuity

The question behind most vendor questionnaires for a firm this size is whether we will still be here, so here is the straight answer:

This document is part of Queantic's vendor packet. The rest of the packet — and the two items we supply on request rather than publish — are listed on the procurement page. Questions about anything in it: ask us directly.
